Hemorrhoid Hygiene

Hygiene is critical to successful healing of hemorrhoids because of their location. Bacteria, like the ever-present E. coli, or (less often) staphylococcus or S. aureus, inhabit the anal cavity.

In a healthy, non-inflamed anus, this bacteria poses no problem because the internal sphincter (between anus and rectum) is intact and tightly closed. In addition, there is adequate blood flow which keeps tissues healthy and resistant to infection.

An inflamed hemorrhoid compromises the tissues and places the person at risk for bacterial infection, leading possibly to anal abscess and fistulae (pus-filled tunnels).

Fortunately, there are many simple ways to maintain good hygiene.

-Direct a stream of water with a squeeze bottle or shower attachment.

-Sit in a sitz bath - sit in bathtub, fill tub with warm water to hips, open anus gently with fingers, and let it soak. Separate stand-alone or toilet attachment sitz baths are available.

-Use a bidet. These are available as free-standing units or toilet attachments.

-Lukewarm water enemas, using a bulb syringe or enema bag - but only if there is no abscess or pain present.

Twice daily, or after each bowel movement, wash with a gentle and non-irritating cleanser such as castile soap or Aloe Vesta made especially for the delicate perineal area. Don't use perfumed soaps that can irritate the anal mucous membranes.

Many rectal irritations can be soothed immediately after a thorough cleansing. Undigested food and or digestive acids in feces can lodge in anal tissue folds. Washing these out can result in dramatic relief of anal itching, swelling and soreness.

Special care must be taken in cleansing external hemorrhoids, which protrude from the anus. External hemorrhoids are especially prone to trapping debris which can cause further inflammation. A direct flow of warm water on the area is the best way to flush away trapped debris. Follow with a sitz bath or warm compress for longer lasting relief.

I heard from a friend of a friend that a hot washcloth applied to the anus is part of that person's daily anal hygiene routine. Don't get it too hot, though! I would keep it about as hot as a facial pack.
